Instructions for completing forms on-line:

Place cursor on gray square and begin typing, e.g. your name, address, membership #, etc.
If you see an arrow pointing down (down arrow (336bytes)) when you click on the gray box, double click the box, move your cursor to the appropriate info line (e.g. under "type of course," move the cursor to ARC, TRC, etc), click on the appropriate info and it will automatically be placed in the block
Save the completed form ("save as") to your desktop
Send the form electronically as an attachment as normal e-mail to the appropriate e-mail address.
